Exploring New Treatments for Chronic Lymphocytic Leukemia and Small Lymphocytic Lymphoma
(CLL/SLL)

A new medicine called Lisaftoclax (APG-2575) is being studied in combination with Acalabrutinib to see if it works better than Acalabrutinib alone.

This clinical trial is for people who have already been treated for chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL) and are still living with their disease.

ClinicalTrials ID NCT06104566

About the Study

About the Study

A Global, Multicenter, Phase 3 Study of Lisaftoclax (APG-2575) for Patients with Chronic Lymphocytic Leukemia (CLL) and Small Lymphocytic Lymphoma (SLL)

This study investigates whether the addition of Lisaftoclax to acalabrutinib therapy can improve treatment outcomes in previously treated CLL/SLL patients.

Enrollment Goal: 440 patients

Fast Escalation
to Target Dose

Lisaftoclax requires just a 5-day dose escalation, compared to up to 6 weeks with other regimens like venetoclax.

This means faster treatment starts and less burden for patients and sites.

Study Groups

Lisaftoclax
Acalabrutinib
Investigational Arm:

Combination therapy with Lisaftoclax (400 mg once daily following a ramp-up phase) and Acalabrutinib

Acalabrutinib

Control Arm:

Acalabrutinib monotherapy

Key Objectives

Primary Endpoint
Progression-Free Survival (PFS)
Secondary Endpoint
Overall Survival (OS)
Objective response rates
Safety and tolerability
Patient-reported outcomes

Key Inclusion Criteria

  • Patients aged 18 or older with a confirmed diagnosis of CLL/SLL
  • Patients must have been on continuous monotherapy with a BTK inhibitor (acalabrutinib) for at least 12 months prior to screening. Monotherapy with BTKi must be within the first three lines of therapy for CLL/SLL.
  • Prior treatment with venetoclax or other Bcl-2 inhibitors is prohibited. Other prior therapies (e.g., anti-CD20 antibodies or chemotherapy) are permitted if the total number of prior lines does not exceed three.
  • Stable disease or partial response to prior acalabrutinib therapy*
  • Adequate organ function (renal, liver, bone marrow)

Key Exclusion Criteria

  • Patients who achieved complete response or experienced disease progression on Acalabrutinib before study entry
  • Prior treatment with other BCL-2 inhibitors such as venetoclax
  • History of Richter’s transformation or other significant medical conditions that may compromise safety
  • Uncontrolled infections or significant cardiovascular issues

FAQ

General Questions
What is the purpose of this clinical trial?

This trial is evaluating the safety and effectiveness of Lisaftoclax (APG-2575) in combination with Acalabrutinib compared to Acalabrutinib alone in patients with previously treated Chronic Lymphocytic Leukemia (CLL) or Small Lymphocytic Lymphoma (SLL).

Who is eligible to participate?

Patients aged 18 years or older with a confirmed diagnosis of CLL/SLL who have been treated with Acalabrutinib for at least 12 months and have stable disease (SD) or partial response (PR) may be eligible. Other medical criteria apply, including liver and kidney function assessments.

How long does the treatment last?

Participants will continue treatment until disease progression, unacceptable side effects, withdrawal from the study, or completion of the study, which may last up to 7 years.

Treatment and Medication
What is Lisaftoclax (APG-2575)?

Lisaftoclax is an investigational drug designed to block BCL-2, a protein that helps cancer cells survive. It is taken orally once daily.
Lisaftoclax is an investigational drug and is not yet approved by FDA.

How is the treatment administered?

Lisaftoclax is taken orally, starting with a 5-day ramp-up phase to gradually increase the dose to 400 mg per day. Acalabrutinib is taken twice daily as a pill.

Will I need to stay in the hospital during treatment?

Most patients will take the medication at home, but those at high risk for Tumor Lysis Syndrome (TLS) may require monitoring in a hospital during the ramp-up phase.

Safety and Side Effects
What are the possible common side effects of Lisaftoclax?

Common side effects include:

  • Fatigue
  • Nausea
  • Diarrhea
  • Low blood cell counts (neutropenia, thrombocytopenia)
  • Tumor Lysis Syndrome (TLS), a serious complication from rapid tumor breakdown
What safety measures are in place?
  • Regular blood tests and clinic visits to monitor your health
  • Tumor Lysis Syndrome (TLS) risk assessment before starting treatment
  • Hydration and medication to reduce TLS risk
  • Dose adjustments if side effects occur
What happens if I experience severe side effects?

The medical team will assess your condition and may adjust your dosage, pause treatment, or discontinue medication if necessary.

Participation and Logistics
Can I withdraw from the study at any time?

Yes, you can withdraw at any time without affecting your standard medical care.

What happens after the trial ends?

Your doctor will discuss ongoing treatment options and whether you may continue receiving Lisaftoclax if it proves clinically beneficial.
